- Our membership plan is not insurance, nor a health plan under the Affordable Care Act. It’s a direct payment arrangement for services, products, and care that we provide, not pre-payment for future services.
- We offer a 30-day money-back guarantee on your membership. If you haven’t used any services or received discounts, you can request a full refund within 30 days of enrollment.
- To access the services and discounts in your plan, payments must be up-to-date.
- Payments for treatments not covered by your plan are due at the time of service.
- Membership benefits cannot be combined with insurance or other discounts.
- Procedures performed by specialists outside of our practice are not included in membership plans.
- Memberships last for 12 months with automatic renewal. You can cancel auto-renewal at any time and will receive a reminder 45 days before your renewal date.
